Confined spaces kill workers every year — and what makes them especially dangerous is that the hazards are often invisible. Oxygen-deficient atmospheres, toxic gases, and engulfment hazards don't announce themselves. OSHA's Permit-Required Confined Space standard (29 CFR 1910.146) exists because workers have died entering spaces to rescue colleagues who had already succumbed to atmospheric hazards — creating multiple fatalities from a single incident. Compliance with this standard is not optional, and the consequences of non-compliance are measured not just in citations and fines, but in lives.
Confined Space Classification
Not every enclosed space is a confined space under OSHA's definition — and not every confined space is permit-required. Getting the classification right is the first and most critical step. A confined space is large enough for a worker to enter and perform work, has limited means of entry or exit, and is not designed for continuous occupancy. A permit-required confined space (PRCS) has one or more of the following: a hazardous atmosphere, a material that could engulf an entrant, an internal configuration that could trap or asphyxiate, or any other recognized serious safety or health hazard. Misclassifying a PRCS as a non-permit space is one of the most common — and most dangerous — compliance failures.

- Identify and inventory all confined spaces at your facility — include tanks, vessels, silos, pits, manholes, tunnels, and crawl spaces
- Evaluate each confined space against OSHA's four PRCS criteria — document your classification decision
- Post danger signs at all permit-required confined spaces: 'DANGER — PERMIT-REQUIRED CONFINED SPACE — DO NOT ENTER'
- Review classifications whenever the space's configuration, use, or surrounding processes change
- Do not rely on historical classifications — verify with current atmospheric testing and hazard assessment
- Document all non-permit confined spaces and the basis for that classification in writing
Written Permit Space Program
OSHA requires employers with permit-required confined spaces to develop and implement a written program before any entry is authorized. This program must describe the employer's procedures for safe entry, the roles and responsibilities of all participants, the equipment to be used, and the rescue and emergency services available. The written program must be made available to employees and their representatives. A program that exists only on paper — without training, equipment, and practiced procedures behind it — provides no protection and creates significant legal exposure.

- Develop a written PRCS program that covers all spaces at your facility — one generic program is not sufficient if spaces have different hazards
- Define the roles of entry supervisor, authorized entrant, and attendant — and ensure each person understands their specific responsibilities
- Include procedures for canceling or suspending entry permits when conditions change
- Specify the equipment required for each space: atmospheric monitors, ventilation, communication, PPE, and retrieval systems
- Review and update the written program annually and after any entry incident or near-miss
- Make the written program available to all affected employees — not just supervisors
Atmospheric Testing & Monitoring
Atmospheric hazards are responsible for the majority of confined space fatalities. Oxygen deficiency, flammable gases, and toxic contaminants can all be present simultaneously — and conditions can change rapidly during entry. OSHA requires that the atmosphere be tested before entry and continuously monitored during entry whenever there is a potential for atmospheric hazards. Testing must be performed by a qualified person using calibrated, direct-reading instruments. The order of testing matters: oxygen content first, then flammability, then toxicity.

- Test the atmosphere before entry — in this order: oxygen content, flammable gases/vapors, then toxic contaminants
- Acceptable entry conditions: oxygen 19.5%–23.5%, flammable gases below 10% of LEL, toxic contaminants below applicable PELs
- Use calibrated, direct-reading instruments — verify calibration before each use and document it
- Test at multiple levels within the space — hazardous atmospheres stratify; a safe reading at the top doesn't mean the bottom is safe
- Monitor continuously during entry whenever atmospheric hazards could develop — not just at the start
- Immediately evacuate and cancel the permit if atmospheric conditions exceed acceptable limits at any point during entry
Entry Permits & Authorization
The entry permit is the operational control document for every PRCS entry. It must be completed before entry begins, signed by the entry supervisor, and posted at the entry point for the duration of the entry. The permit documents the space being entered, the purpose and duration of entry, the authorized entrants and attendants, the hazards present and the measures taken to control them, the atmospheric test results, the communication procedures, and the rescue services available. Permits must be retained for at least one year after each entry to allow for program review and improvement.
Key Action Items
- Complete the entry permit before any worker enters the space — no entry without a signed, current permit
- Post the permit at the entry point and ensure all entrants and the attendant have reviewed it
- Cancel the permit immediately if conditions change, an emergency occurs, or the entry is completed
- The entry supervisor must verify that all listed conditions are met before signing — this is not a rubber stamp
- Retain cancelled permits for at least one year — review them annually to identify trends and improve procedures
- Never reuse a permit — each entry requires a new permit, even if the same space was entered the day before
Attendant Duties & Rescue Planning
The attendant is the entrant's lifeline. OSHA requires that at least one attendant remain outside the permit space at all times during entry — and that attendant must be trained, equipped, and authorized to initiate rescue. The attendant must maintain continuous communication with entrants, track who is inside, monitor for hazardous conditions, and order evacuation when necessary. Critically, the attendant must never enter the space to perform rescue — non-entry rescue using mechanical retrieval equipment is required whenever feasible. Attendant-attempted entry rescues have turned single fatalities into multiple fatalities.
Key Action Items
- Station at least one trained attendant outside the space for the entire duration of every entry — no exceptions
- The attendant must maintain continuous two-way communication with all authorized entrants
- Provide retrieval systems (tripod, winch, lifeline) for non-entry rescue whenever the vertical distance makes it feasible
- The attendant must never enter the space to perform rescue — summon trained rescue services instead
- Establish and communicate the rescue plan before entry begins — including how to contact emergency services and the estimated response time
- Evaluate whether on-site rescue capability is needed based on response time of external services — if EMS cannot respond in time, on-site rescue must be trained and equipped
Training Requirements for All Roles
OSHA requires that all employees involved in permit space operations be trained to the level required by their role — and that training be provided before initial assignment, before a change in assigned duties, and whenever there is reason to believe the employee has not retained the required knowledge or skills. Training must be documented with the employee's name, the trainer's signature, and the dates of training. Authorized entrants, attendants, and entry supervisors each have distinct training requirements — and the training must be specific to the actual spaces and hazards at your facility.

- Train authorized entrants on hazard recognition, atmospheric testing, permit procedures, and emergency evacuation
- Train attendants on their specific duties, communication procedures, non-entry rescue, and when to summon emergency services
- Train entry supervisors on permit authorization, hazard control verification, and permit cancellation authority
- Conduct training specific to the actual spaces and hazards at your facility — generic training is insufficient
- Document all training with employee name, trainer signature, and date — retain records
- Retrain whenever an employee's performance indicates a knowledge or skill deficiency — don't wait for an incident
