Forklifts are among the most dangerous pieces of equipment in any warehouse, distribution center, or manufacturing facility. OSHA estimates that nearly 85 forklift fatalities and 34,900 serious injuries occur in U.S. workplaces every year — and the vast majority are preventable. OSHA Standard 29 CFR 1910.178 requires formal operator training, evaluation, and certification for every powered industrial truck operator. But compliance alone isn't enough. The employers who truly protect their people go further: they build daily inspection habits, enforce traffic management rules, and create a culture where no one operates a forklift without the skills and awareness to do it safely.
Operator Training & Certification Requirements
OSHA 29 CFR 1910.178(l) requires that every forklift operator be trained and evaluated before operating a powered industrial truck — and that training must be specific to the type of truck being operated and the hazards of the workplace. Generic online-only training does not meet this standard. Operators must receive both formal instruction (classroom or computer-based) and practical, hands-on training in the actual work environment. After training, a qualified evaluator must observe each operator performing the tasks required in the workplace and certify that they are competent.

Pre-shift inspections are not a formality; they are the primary mechanism for catching mechanical failures before they cause an incident. Operators must be trained to conduct these inspections and empowered to remove a truck from service without fear of pressure to keep it running.

Load Handling & Stability
Tip-overs are the leading cause of forklift fatalities. The stability of a loaded forklift depends on the load weight, load center, mast tilt, travel speed, and surface conditions — and these factors interact in ways that are not always intuitive. Operators must understand the forklift's data plate, the concept of the stability triangle, and how load placement affects the center of gravity. Overloading, improper load placement, and traveling with elevated loads are among the most common — and most deadly — forklift errors.

- Never exceed the rated load capacity shown on the forklift's data plate — account for load center distance
- Carry loads as low as possible during travel — typically 6–8 inches above the floor
- Tilt the mast back when traveling with a load to improve stability
- Approach loads squarely — never pick up or deposit a load at an angle
- Slow down on turns, ramps, and uneven surfaces — speed is a primary factor in tip-overs
- Never allow passengers to ride on forklifts unless a designated seat with a seatbelt is provided
Pedestrian Safety & Traffic Management
Pedestrian-forklift collisions are a leading cause of serious injuries and fatalities in warehouse and manufacturing environments. OSHA requires that employers establish safe speeds, designate traffic lanes, and implement measures to separate pedestrians from forklift traffic wherever possible. Physical barriers, floor markings, warning signs, and procedural controls all play a role — but the most effective protection is a well-designed traffic management plan that eliminates the conditions where pedestrians and forklifts share the same space.

- Designate and clearly mark pedestrian walkways and forklift traffic lanes with floor paint or barriers
- Post and enforce speed limits in all forklift operating areas
- Install mirrors at blind intersections and corners to improve visibility

Refueling, Battery Charging & Maintenance Safety
Refueling and battery charging operations introduce fire, explosion, and chemical exposure hazards that require specific controls. LPG cylinder changes, propane refueling, and lead-acid battery charging all have OSHA requirements that are frequently overlooked. Battery charging areas must be ventilated to prevent hydrogen gas accumulation, and open flames or sparks must be prohibited in these areas. LPG cylinders must be stored and handled according to NFPA 58 requirements. These are not minor administrative details — they are the controls that prevent fires and explosions.

- Designate and ventilate battery charging areas — hydrogen gas from charging batteries is explosive
- Prohibit smoking, open flames, and sparks in battery charging and LPG refueling areas
- Provide eyewash stations and PPE (face shield, acid-resistant gloves) in battery charging areas
- Store LPG cylinders upright, secured, and away from heat sources and ignition points
- Train operators on proper LPG cylinder exchange procedures — including leak detection
- Perform all maintenance and repairs only when the forklift is shut down and locked out
Incident Investigation & Continuous Improvement
Every forklift incident — including near-misses — is a signal that something in your system needs to change. OSHA requires that incidents be investigated and that operators involved in accidents be evaluated before returning to operation. But the most safety-conscious employers go further: they treat near-misses as free lessons, conduct root cause analyses that look beyond operator error to identify systemic failures, and use incident data to drive continuous improvement in training, equipment, and facility design.
